CLINICAL REFERRALS

If you are a healthcare professional seeking detailed treatment information we invite you to contact us directly so we can connect you to your local Treatment Consultant.

If you would like to refer a patient to our program, please complete the referral form and email or fax it in to us when complete. We work collaboratively with referring practitioners to ensure consistent and continuing care pre-admission and pre-discharge.

EMPLOYERS, INSURERS, AND DISABILITY MANAGEMENT GROUPS

If you are looking for a trusted treatment program for your employee or claimant that can also incorporate return to work into treatment and discharge planning, we invite you to connect with us directly so we can discuss the unique return to work needs and goals of your case.

OTHER THIRD PARTY

Veterans Affairs Canada (VAC) and Royal Canadian Mounted Police (RCMP) health identification cards accepted. If you are seeking detailed treatment information we invite you to contact us directly so we can put you in touch with your Treatment Consultant.
